Physical Therapy After Hip Replacement

[Therapist] Good morning, Mrs. White. I'm your physical therapist, James. We're going to start your rehabilitation today, just two days after your hip replacement. [Patient] Already? I'm still sore. [Therapist] I completely understand. But early mobilization is actually one of the most important factors in your recovery. We'll take it slow and stay within your comfort level. [Patient] What are we going to do today? [Therapist] Three things: ankle pumps to prevent blood clots, gentle leg slides to maintain range of motion, and we're going to practice sitting on the edge of the bed and standing with a walker. [Patient] I'm nervous about putting weight on it. [Therapist] That's completely normal. Your surgeon approved weight-bearing as tolerated, which means you can put as much weight on it as feels comfortable. The prosthetic joint is very strong. [Patient] Will I need the walker for long? [Therapist] Most patients use the walker for two to four weeks, then transition to a cane, and then walk independently. Everyone progresses at their own pace. [Patient] Are there movements I should avoid? [Therapist] Yes, there are three important precautions. Don't bend your hip past ninety degrees — so no deep bending. Don't cross your legs. And don't twist your operated leg inward. These precautions prevent dislocation of the new joint. [Patient] How long until I can drive again? [Therapist] If it's your right hip, typically six to eight weeks. You need to be off pain medication and have enough strength and reaction time to brake safely. [Patient] What's the overall recovery timeline? [Therapist] Most patients feel significantly better by six weeks, reach major milestones by three months, and continue improving for up to a year. Consistent physical therapy is the key to the best outcome.
